New Public Safety Officer Benefits Form Posted April 29, 2011 A new beneficiary form that is accepted for the Public Safety Officer Benefits program is now available. This form
can be used in place of the original six page form. All Fire Departments should print this form on their own
letterhead, have each volunteer firefighter complete the form in private, and maintain the forms in a secure file in
individually sealed envelope at the station. These forms should be reviewed and updated annually.

Otis Wyatt Awarded Firefighter of the Year Award Posted January 23, 2011 The Rhode Island State Firemen's League has selected Retired Chief Otis Wyatt as the 2010 recipient of the
annual Firefighter of the Year Award. President Richard DiCarlo presented Chief Wyatt with the award during the annual Presidents Dinner.
Chief Wyatt has served the fire service for the past sixty-eight years. He began his fire service journey in 1942 at
the age of fifteen with the Lakewood Fire Department in Warwick in 1942 and remained an active member until
being hired in 1952 as the fourth career firefighter for the City of Warwick. He retired in 1980 with the rank of
Deputy Chief and was appointed Chief of the Narragansett Fire Department. He retired from Narragansett in 1992
and then served as the Fire Chief in Tiverton. Otis has faithfully served as a loyal and active member of the Rhode
Island State Firemen's League for the past fifty-four years and has served on numerous committees and held several positions including the office of President.
